POSITION TITLE: Adult Rehabilitative Mental Health (ARMHS) Mental Health Practitioner
DEPARTMENT: Adult Community Support Program
GENERAL HOURS
OF WORK: Full-Time; Daytime Hours Monday through Friday
EXEMPT/NON EXEMPT: Exempt
SUPERVISOR: Director of Operations
POSITION SUMMARY: Provides professional, client-centered rehabilitative services and teaching skills to individuals with serious mental illness or traumatic brain injury. This position will be responsible for providing services which will enable the recipient to develop and enhance psychiatric stability, social competencies, personal and emotional change and independent living and community skills.
EDUCATION, EXPERIENCE AND SKILLS REQUIRED:
Mental Health Practitioner Status obtained through education and/or experience.
Must have good verbal, written, organizational, interpersonal and computer skills.
ESSENTIAL FUNCTIONS:
Performs all duties in accordance with the policies and procedures of the Center.
Develop and implement core competencies of DHS: Motivational interviewing, IMR Recovery Philosophy.
Ensure routine safety plan is completed
Provide instruction, assistance, and support to the client and provide them with self-advocacy skills to promote independence.
Aid in mental illness symptom management skills.
Assist clients to work on goals and objectives that result from the treatment plan.
Provide medication monitoring.
Encourage interpersonal communication skills.
Explain relapse prevention skills.
Provide opportunities for new experiences in the community or home.
Maintain confidentiality in all aspects of the job.
Follow individual treatment plans (ITP). Complete Functional Assessment and ITP within 30 days of meeting a new client, review/update every 180 days.
Ensure Treatment plan, assessments and corresponding documentation are updated as required by ARMHS policy.
Coordinate services with other agencies, institutions, employers, landlord, or family members in order to alleviate problems and promote independence. Maintain a good working relationship with all other agencies.
Support new team members by providing client information training.
Participate in required agency meetings and in supervision meetings. Please do not be late.
Complete professional training according to ARMHS regulations and to promote professional growth and development.
Compile and complete agency documentation in a timely manner. ARMHS is required to do concurrent documentation with all clientele notes should be done before you leave clients home; if they are mistrustful or fearful have they write the note with you, read it back to them so they become comfortable with the state required documentation.
Achieve productivity expectations.
Complete training and requirement of IMR and provide to all clients.
Positively support and promote recovery to clients.
Adhere to SWMHC Teamwork and Professionalism by maintaining confidentiality, having a positive attitude, treating others with respect, being flexible, accepting responsibility and utilizing open and effective communication to SWMHC staff and clientele.
Above all treat clientele with respect, use appropriate tones and body language, mirror back what you have heard them say to you, have them mirror back what they have heard from you to lower confusion and create a good working relationship.
Complete year Relias Learning annual trainings.
ARMHS Mental Health Practitioners must receive continuing education: at least 30 hours every two years in the areas of mental illness and mental health services
OTHER RESPONSIBILITIES:
Is sensitive and responsive to management needs such as data collection and program evaluation.
Recruitment and training of community workers as necessary and/or required.
Ability to travel as required.
Performs other work-related duties as assigned.
REQUIRED PHYSICAL ABILITIES:
While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; and talk or hear.
The employee frequently is required to travel to all SWMHC facilities in the catchment area, ambulate within and between departments; reach with hands and arms; stoop, kneel, crouch, or crawl; and taste or smell.
The employee is occasionally required to sit and climb or balance.
The employee must occasionally lift and/or move up to 40 pounds.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:
Must possess the required level of skills and abilities to read, write, speak, and apply mathematical concepts and sound reasoning to successfully perform the essential duties and responsibilities of this job.
Must be able to perform basic operations on a computer within the Windows operating system.
This position requires problem solving skills, critical thinking skills, and the ability to multi-task.

WORK ENVIRONMENT:
May be exposed to communicable diseases when working in a health care environment.
The employee is occasionally exposed to wet and/or humid conditions and outside weather conditions.
The noise level in the work environment is usually moderate.
