Are you passionate about HIV Medicine/Infectious Diseases and looking for an ambulatory clinical pharmacist position in an academic setting? UVA Health is seeking a pharmacist team member to support our Ryan White HIV and Infectious Diseases Clinics.
The Ambulatory Care Clinical Pharmacist for HIV and Infectious Diseases collaborates to provide medication management and patient education in order to optimize clinical outcomes. This position primarily supports a large multidisciplinary team within the UVA Health Ryan White HIV Clinic which cares for approximately 1,000 patients living with HIV. Clinical activities include but are not limited to: 1) rapid initiation of antiretroviral therapy (ART), 2) cabotegravir/rilpivirine coordination, 3) complex ART optimization, 4) assistance with polypharmacy, drug-drug interactions, adverse drug events, dosing adjustments, and primary care management for patients with HIV, 5) medication adherence counseling, and supporting medication access technicians. In partnership with HIV clinic leadership, there are opportunities for this team member to build the cabotegravir/rilpivirine (and long-acting injectable HIV treatment and prevention) program in addition to an HIV prevention program. This HIV clinic is regularly recognized by the Virginia Department of Health and the Health Resources and Services Administration for best practices, and if desired, opportunities to generate and present scholarly work are possible. Similar drug therapy expertise for other Infectious Diseases clinics, including General Infectious Diseases, Transplant Infectious Diseases, Travelers Clinic, Tuberculous and Non-Tuberculous Mycobacterial Diseases Clinic, and Musculoskeletal Infectious Diseases Clinic, may be provided upon request. There is also opportunity for collaboration with inpatient ID pharmacists to support clinic visits for patients followed by the Outpatient Parenteral Antimicrobial Therapy (OPAT) service. Finally, outpatient antimicrobial stewardship activities may be pursued in this role depending on candidate interest. Education-related expectations include: serving as a rotation and/or project preceptor for APPE students and residents (PGY1, PGY2 Ambulatory Care, PGY2 Infectious Diseases), and didactic lectures/in-services for staff and trainees (Infectious Diseases fellows, Internal Medicine Residents who have continuity clinic in the HIV clinic). This individual will promote UVA Specialty Pharmacy's culture of caring and communication, which fosters patient empowerment through education and support.
The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

Position Compensation Range: $52.46 - $83.94 Hourly
MINIMUM REQUIREMENTS
Education: Bachelor's in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.
Experience: PGY-1 residency required or equivalent clinical experience.
License/Certification: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged.
