    Description:

    POSITION PURPOSE

    A Discharge Lounge RN provides individualized, high-quality nursing care to patients of all ages. Uses a team approach to coordinate care at transfer and discharge. Independently delivers nursing care using the Nurse Practice Act, MercyOne Medical Center policies, nursing standards, ethical principles, and professional judgment. Ensures the delivery of high quality, cost-effective health care consistent with the mission of MercyOne.     ESSENTIAL FUNCTIONS

    Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior. Assesses plans and implements nursing care for patients of diverse ages while ensuring patient comfort. Practices safe patient handling techniques and is proficient with aseptic techniques. Administers nursing care to both acute and ambulatory patients including assessment, planning, implementing and evaluating of care plans and orders in coordination with providers. Interacts with patients and families, identifying and responding to educational and emotional needs. Provides patient/family or caregiver education and counseling in recognition and solution of health problems. Initiates emergency support measures (i.e. CPR, protecting patients from injury). Maintains open and positive communication with staff, providers, and other departments. Participates in problem solving with staff, providers, patients, families and other departments/nursing areas. Maintains knowledge of equipment setup, maintenance, and use (i.e. vital machine equipment, and other devices). Administers medications and provides treatment as ordered by a physician. Common tasks include but are not limited to: receiving handoff report, facilitating discharge (contacting nurses and physicians, arranging follow up care, etc.), administering discharge instructions, and tending to patient needs. Demonstrates hands-on knowledge of problems and diagnoses to provide thorough dishcharge teaching. Demonstrates ability to quickly acquire new knowledge. Provides patient/family or caregiver education and counseling in recognition and solution of physical, emotional, and environmental health problems. Proficient in the use of the clinic and hospital EMRs for information gathering and communication. Coordinates with offices and scheduling departments to ensure accurate follow up appointments. Collaborates with ordering providers to clarify orders. Maintains the physical environment of the department in a clean, well-stocked, orderly manner.

    MINIMUM QUALIFICATIONS

    Possession of a current or compact state licensure as a registered nurse issued/defined by the State of Iowa Basic Life Support (BLS) for the Healthcare Provider certified or obtained within thirty (30) days of hire. Proof of completion of Mandatory Reporter abuse training specific to populations served within thirty (30) days of hire. Must possess excellent verbal and written communications skills, presentation skills, advanced analytical skills, be well organized and maintain a highly professional manner and appearance. Strong interpersonal, consultative and relationship-building skills to initiate and develop productive collaborative partnership with all levels across the organization. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.

    PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

    Must be able to set and organize one's own work priorities and adapt to them as they change frequently. Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles. Must possess the ability to comply with Trinity Health policies and procedures. Must be able to effectively analyze, communicate and respond. Has potential for exposure to infectious diseases, hazardous waste and chemicals. Vision: Preparing and reading written documentation, and the use of multiple computer systems, fine detail for patient care (IV starts), and peripheral vision. Talking and hearing while exchanging information both in person and by phone. Able to respond to audible alarms. Lifting, kneeling, bending, standing, pushing, pulling while delivering direct patient care or utilizing equipment. Exerting up to 100 lbs. of force occasionally and/or up to 40 lbs. of force frequently, and/or up to 20 lbs. of force constantly to move objects.
